A website called The Hospital Safety Grade scores hospitals on how safe they keep their patients from medical errors and avoidable injuries, accidents, and infections. A report written by The Leapfrog Group, a nonprofit organization dedicated to examining the U.S. healthcare system, wrote an article about hospital safety and hospitals that are improving their safety scores. Many hospitals made drastic improvements, but over 150 earned a "D" or "F" score.
